Empower Minnesotans in crisis to access stable housing and critical supports through expert consultative referrals, enabling swift, effective transitions toward self-sufficiency.
Strategic Objectives
Provide Expert Intake & Needs Assessment
Conduct thorough consults (in-person or virtual) to assess each clients circumstances identifying barriers in housing, employment, recovery, or health.
Deliver Tailored Referral Plans
Create individualized referral maps: connecting clients to local programs, resources, and support providers best suited to their needs and situation.
Facilitate Coordination with Minnesota Systems
Link clients to key systems such as Coordinated Entry, 211, Housing Stabilization Services, and PATH-lik e initiatives, ensuring streamlined access across agencies.
Offer Ongoing Navigation & Follow-Up
Provide case monitoring to ensure clients successfully engage with referred services, and adjust guidance as circumstances evolve.
Build Community Partnerships
Evaluate Impact & Improve Service Quality
Develop robust relationships with service providers statewide housing agencies, workforce development, mental health organizations, and disability services to broaden referral accuracy and speed.
Track metrics like referral success rate, client housing outcomes, and client satisfaction, using data to refine processes and advocate for systemic improvements.
